PARENTAL CONSENT AND PERMISSION
for all Participants under the age of 18 years
Please read carefully all the contents of this Agreement before complete the form below
For Security Purposes: All Parent/Guardians MUST read this Entry Rules and Regulations document. Parental/Guardian consent MUST be obtained BEFORE AGREEING to register for the Competition (Adults will be contacted in advance to verify validity) An Adult will be required to accompany the Finalist to the Rehearsals, any activities and Grand Finals Event.
ELIGIBILITY:
The contest is open only to aspiring models who do not have major agency representation. If the participant is already signed to an agency or has representation then this will prohibit the Model from taking place, except if the agency gives written permission that they give authority to the Model to participate.
AGE:
The Competition is open to all legal male and female residents between the age of 6-30 years by the time of the Grand Finals event on In November 2016 (Date To Be Confirmed). The Divisions are as follows:
Junior Model Of The Year - 6-10 years
Young Model Of The Year - 11-16 years
Senior Model Of The Year - 17-30 years
Senior Plus Model Of The Year - 31-50 years
Those taking part who under the age of 18 years will be required to obtain consent and parental or guardian permission before attending the Audition or entering the Competition.

Fashion 4wardz reserves the right to substitute prizes of equal or greater value. There will be no substitution of prizes by prizewinners. Prizes may not be redeemed for cash value. Prizes are non-transferable. All prizes will be awarded, provided a sufficient number of qualifying entries are received. No warranties are made with respect to prizes.
JUDGING:
The Judges Panel will be looking for Models who show exceptional skills, quality presentation and potential. Judge’s Panel’s decision is full and final. Your Scores will be announced LIVE at the Grand Finals Show.
FALSIFIED INFORMATION:
Finalists must not provide falsified information or details on their Registration Form. If it is established that a contestant has provided false information then this will lead to instance dismissal from the event and striping for the Title. The Finalist will be require to return all prizes awarded and this will be given to the 2nd Runner Up who will be the successor to the Title.
ADULT CHAPERONE:
All Finalists under the age of 18 years will be required to have one (1) ONLY Adult Chaperone with them at all rehearsals, workshops, events and backstage at the event. Under no circumstance are they permitted to come to any gathering by themselves. Adult members will be required to sign in on arrival at all events.
BEHAVIOR:
Finalists MUST abide by the Rules and Regulations herein and should, at all times, display first-rate manners, etiquette and team spirit and exercise sportsman-like behavior to all other Finalists, members of staff and all associated. BRE Productions International Inc. has a zero tolerance policy towards profanities, racist, sexist, homophobic / bigoted comments, harassment, obscene or abusive language or gestures. Fashion 4Wardz is a family oriented event and under no circumstances will any of the above behaviors be permitted.
FELONIES OR CONVICTIONS:
Finalists MUST NOT have a CRIMINAL RECORD or have been involved in any serious illegal activities that will, in any way, dishonor the name of the event. All Finalists will undergo a Background Check to establish if they have been involved in any criminal activities, felonies or serious criminal activities that may bring the organization into disrepute. *
RELEASE CONTENT AGREEMENT & CONSENT :
Finalist will give permission and authorize for BRE Productions International, Inc/Fashion 4Wardz to videotape, audiotape, photograph, record, edit or otherwise reproduce my performance, and use it in the formats and for the promotional purposes only. Finalists will agree to indemnify and hold harmless B.R.E Productions International, Inc/Fashion 4Wardz, their employees and any representatives together with the venue or location where the event will be staged against any and all claims arising out of their presentation.
PHOTO RELEASE:
Finalists will give BRE Productions International, Inc/Fashion 4Wardz permission to use their likeness for promotional purposes for the event only.
TITLEHOLDER'S RESPONSIBILITY:
Finalists will agree that, if they fail to operate or live up the expectation of a Fashion 4Wardz Titleholder or adhere to the rules, regulations and terms or obligations hereof, that this could result in your disqualification as a Titleholder together with the immediate return of all prizes awarded and cash prizes. Furthermore, should the Titleholder fail to operate, live up to the expectation or does not conduct themselves commensurate with that of a Fashion 4Wardz Titleholder, or does not adhere to the rules, regulations and terms or obligations hereof, then this could result in their disqualification as a Titleholder together with the immediate return of all prizes awarded and cash.
LIABILITY WAIVER:
(Please read carefully): BRE Productions International, Inc/Fashion 4Wardz and its directors, representatives and associates will not be held responsible for any loss, damages, theft or injury to Contestants taking part in our events
REHEARSALS, MEETINGS AND WORKSHOPS:
Finalists MUST attend all rehearsals, workshops and publicity events to secure their participation in the event. Constant lateness and absences will result in the Contestant being disqualified from the event.
FINALIST'S RESPONSIBILITY:
It will be a MANDATORY REQUIREMENT for all Finalists to sell a minimum of 2 (two) standing tickets for Grand Finals event equal to $60 (Sixty Dollars) and sell a minimum of just $45 (Forty Five Dollars) worth of advertising in the FASHION 4WARDZ SOUVENIR BROCHURE. If you are not able to sell tickets or advertising space then you will be responsible for the $60 ticket sale and $45advertising sales revenue that selling the above would cover. Failure to adhere to the above will result in immediate disqualification and non-return of any fees paid.
TOP 3 SELECTION:
The competition will be judged by an esteemed panel of Judges who will select the Top 3 Models - 3rd Place, 2nd Runner Up and Winner.
TITLE HOLDER'S COMMITMENT:
The Winners of Fashion 4Wardz will embody all the positive attributes required by the organization and should present the organization in their best possible light. They will hold their title for a period of 1 (one) year and must be available to attend the next year’s Grand Finals event.
NOTICE:
Finalists who are not able to participate in the Competition MUST give at least 4 week’s written notice prior to BRE Productions International, Inc and Fashion 4Wardz, so as to give the opportunity for another contestant to take part. The Registration Fee will not be refunded or transferred.
LIMITATION OF LIABILITY; RIGHTS OF USE; MISCELLANEOUS:
All finalists and their parents/legal guardians (if finalist is 18 years and under) will be required to sign and return an Affidavit of Eligibility and a Liability and Publicity Release on qualifying to be a Finalist in the Competition. In the event that a properly completed Affidavit and Release are not returned within 14 days of notification, or notification is returned as undeliverable, the Finalist will be disqualified and an alternate Finalist will be selected.
PUBLICITY:
BRE Productions International, Inc and Fashion 4Wardz will have permission to use the participants name, likeness, photographs, video recordings for advertising, promotional and publicity purposes without additional compensation except where prohibited by law.
RELEASE WAIVER FORM:
All entrants will also be required to sign a Release Form giving permission to use the above.
VALUABLES:
BRE Productions International, Inc And Fashion 4wardz organization will not be held responsible for any damage or loss of property that may arise. It is the sole responsibility of the Finalist to ensure that all valuables and garments are supervised throughout the day.
RESPONSIBILITY:
BRE Productions International Inc. and Fashion 4Wardz and their respective affiliates, parents, subsidiaries, directors, officers, employees and all other entities associated with the development and execution of this Competition shall not be held responsible or liable for injury, loss or damage of any kind resulting from participation in this event.
TICKETS:
TICKET & AD SALES: It will be a MANDATORY REQUIREMENT for All Finalists to sell a minimum of 3 (three) standing only tickets which is equal to $60 s for both the Preliminary Competition and the Grand Finals Event and also sell a minimum of $100 worth of advertising in the Souvenir Brochure and, in the event that they are not able or have not been able to sell the sufficient amount requested, that the Finalists will be solely responsible to pay the balance. The Finalist should also understand, once again, as this is a mandatory requirement and that failure to do so will result in disqualification without the refund of any fees. *Tickets for the Grand Finals Event in November - ADVANCE TICKET PRICES:

FINALIST DISQUALIFICATION:
BRE Productions International Inc. and Fashion 4Wardz also reserve the right in its sole discretion to disqualify any entrant who fails to comply with these Official Rules, who attempts to enter the Contest in any manner or through any means other than as described in these Official Rules, or who attempts to disrupt the Contest or to circumvent any of these Official Rules.
ACTS OF NATURE:
BRE Productions International, Inc. And Fashion 4wardz will not be held responsible in the event that an act of nature, severe/bad weather or power failure causes the event to be canceled or postponed.
Participants agree to be bound by these rules and the decisions of the judges, which are final. Contestants, finalists and winner agree to adhere to the rules of this contest.

CASTING CALLS
- All Models wishing to be considered as a Finalist for the FASHION 4WARDZ MODEL OF THE YEAR 2015 COMPETITION will need to compete in one of our "FASHION 4WARDZ CASTING CALL" by applying online at http://www.fashion4wardz.com/model--designer-registration-forms-2015.html . The non-refundable, non-transferable Casting Fee is just $5 (Five Dollars only) paid in advance via the Fashion 4Wardz website or $10 during the calendar week of the Casting events, once you have paid, then we will contact you with the date, time and relevant Audition appointment schedule.
- All Participants will need to wear plain t-shirt, jeans or leggings and females aged 17-50 years of age should be prepared to walk in high heels. Please note: Models age between 6-16 years will need to dress age-appropriate.
- Participants should also be able to provide Fashion 4Wardz with either a comp card or 2 photographs which should consist of a head shot, a three quarters shot and a full length that defines your physique.
- All Models should wear natural make-up, have clear clean and not have excessive tattoos or piercings.
- All Models will be required to demonstrate a "High Fashion" and "Signature Walk" catwalk and undertake a brief interview in the presence of our esteemed Judges Panel from the world of Fashion, Media and Entertainment.
- Models who are successfully selected as a Finalist will be awarded a "GOLD FINALIST INVITATION SLIP" which will enable them to go through to the "FASHION 4WARDZ MODEL OF THE YEAR GRAND FINALS 2016" in November 2016 at the Georgia World Congress Center in Atlanta, GA
PARTICIPANTS CONFIRMATION OF ACCEPTANCE
- Once you have received your "GOLD FINALIST INVITATION SLIP", then you will need to confirm acceptance within 10 (ten) business days to accept and pay the mandatory non-refundable, non-transferable FINALIST HOLDING FEE of $25 (Twenty Five Dollars Only) to hold their place as a Finalist.
- Finalists will also be required to pay and additional FINALIST REGISTRATION FEE of $100 (One Hundred Dollars Only) by September 14 to do so will result in your ineligibility to participate as a Finalist. Fees received after this date will incur an additional late fee of $45 (Forty Five Dollars Only)
- Your Sponsorship Registration Fee will include access to the exclusive FASHION 4WARDZ MODEL CAMP, your own limited edition "FASHION 4WARDZ T-SHIRT and BANDANA" plus a $2,400 Scholarship towards your fees to attend the incredible FUTURE STAR STUDIOS in Manhattan, NY which specializes in dance, drama, acting, singing, performing, modeling and all aspects of the entertainment and modeling industry!
- A Payment Installment Plan arrangement is available for you to pay over 2 separate installments at a extra charge of $25 per transaction so please contact Fashion 4Wardz for more details.
- You can also use the PayPal Credit service to help with budgeting costs. Please see banner at the end of this page for further details.
- It will be mandatory for all Finalists to sell a minimum of 5 (five) tickets for the event at $25 per ticket. If you are not able to sell these tickets then you will be responsible for the $100 amount that selling the tickets would cover.
- It will also be mandatory for each Finalist to sell a minimum of 1/2 page advertising in the "FASHION 4WARDZ MODEL OF THE YEAR MAGAZINE". If you are not able to sell the required advertising space then you will be responsible for the $100 amount that selling the tickets would cover.
- Please note: A minimum of 10 Participants are required for each Division.

Judges decisions are final. Any Contestant, relative or guest displaying unsportsman-like, crude or unsavory behavior will be disqualified and monies will not be refunded. All returned checks will incur a $45 fee. By completing this online Registration, you are agreeing to the regulations and agreement of BRE PRODUCTIONS INTERNATIONAL, INC AND FASHION 4WARDZ and will do what is necessary to observe our statues. In the event that this is not adhered to, the Finalist will be disqualified and any prizes awarded will be returned forthwith to BRE PRODUCTIONS INTERNATIONAL, INC AND FASHION 4WARDZ immediately. You also understand that all Registration fees are NON-REFUNDABLE should you wish to no longer take part in the Event. Your personal information supplied herein will not be sold, transferred or shared with any outside organization.
